POC form and would like a POC form to file a claim, please contact Far West at the toll free
number or mailing address indicated on this Web Site (How to Reach Us). Please note that the
POC forms will not be available from the Nebraska Department of Insurance.
The POC forms are provided on this Web Site as a convenience/informational purposes only and are
intended for viewing purposes only. Only claims filed on prescribed POC forms received by Far West
